Waiver and release of liability for AKA San Antonio/B.South BJJ

I {SIGN BELOW} hereby agree to the following:

1. That I am participating in the training programs, exercises, group fitness, Jiu Jitsu, Boxing and other mixed martial arts classes and private training offered by Bobby Southworth and other coaches employed by AKA San Antonio/B.South BJJ.
2. I understand that it is completely my responsibility to consult with a physician prior to and regarding my participation in ANY type of fitness/mixed martial arts related programs. I represent and warrant that I am physically fit and have no medical conditions that will prevent my full participation in classes offered and or private training.
3. In consideration of being permitted to participate in any fitness classes, boxing, jiu jitsu, mixed martial arts or private training I agree to assume full responsibility for any risk, injuries (including death) or damages known or unknown, which I might incur as a result of participating in any or all activities/ classes and or private training in the program.
4. In consideration of being permitted to participate in any fitness classes, boxing, jiu jitsu, mixed martial arts or private training, I agree to waive any claim I may have against AKA San Antonio/B.South BJJ, Bobby M. Southworth, building owner(s), landlords, property managers, HOA, insurers, or any AKA San Antonio/B.South BJJ coaches or instructors - for any injuries (including death), damages that I may sustain as a result of participating in any or all activities/ classes and or private training in the program.
5. I, my heirs or legal representatives forever release, AKA San Antonio/B.South BJJ, Bobby M. Southworth, building owner(s), landlords, property managers, HOA, insurers, or any AKA San Antonio/B.South BJJ coaches or instructors -ANY injury or death caused by their negligence, instruction or other acts.

I have read and fully understand the above waiver of liability and fully understand the contents. I voluntarily agree to the terms and conditions stated above.
